- Name and title of each State Council Member, i.e. chair, vice-chair, secretary, member, etc.
- Description of the group they represent, i.e. Executive branch, Judiciary, victims, etc.
- The date of appointment and length of term
Within 60 days of a State Council meeting, each member state will provide the National Office with a copy of the meeting agenda and minutes.
+ State Council membership is to be submitted using the State Council Reporting form.
+ States may submit meeting minutes or other related documents to the National Office.
