Revised Compact Forms

The Training Committee has revised a few of the ICAOS forms to conform to the recent Rule amendments which became effective January 1, 2008.

The official forms can be accessed via the Compact Forms Page. As a reminder, you must be logged in to view or download the forms from the website.

Below is a list of the forms and changes which have occurred.


Transfer Request:

  • Sending and receiving state sex offender registration boxes were added to conform to the definition of "sex offender."
  • Under offense information, "other" was changed to "deferred."
  • Under the mandatory resident reason for transfer, "How long?" was removed.
  • The check list of attachments no longer lists the consent to random drug and alcohol testing form or the authorization for medical information form. The additional requirements for sex offender transfers were added under the "if available" check list.

Reply to Transfer Request:

  • Sending and receiving state sex offender registration boxes were added to conform to the definition of "sex offender."

Request for Reporting Instructions:

  • Sending and receiving state sex offender registration boxes were added to conform to the definition of "sex offender."
  • Reminder was added under reason #1 for sex offender probationers living in the receiving state at the time of sentencing to remain in the sending state until receipt of reporting instructions.
  • Under offense information, "other" was changed to "deferred."
  • "Age of victim" was added to description under offense for sex offenders.

Offender Violation Report:

  • An "Absconder" choice and note to "check all that apply" were added to the "type of report" section.
  • Recommendations were reordered and clarified the return option.
  • New fields were added for the requirements for absconding offenders.
  • Attachment section was added to specify what type of documentation is attached to a violation report.

Although it is important that all states use the official and most up-to-date forms, the committee and national office realize that states will need time to inform their field offices of the updates and ensure their local drives and systems have the up-to-date versions of the forms. Therefore, 60 days will be allowed as a grace period upon implementing the new forms.
